Enrolment

Please provide as much information and detail as possible as it will assist our team in supporting your child. Required responses are indicated by green border on the inputs.

Does your child have any medical issues or concerns we need to be aware of (including allergies)?

Would you be okay with us taking photos and videos during our sessions to share on our website and social media pages? We love capturing the fun and excitement of our sessions and would love to showcase your child's progress.

Terms & Conditions

Payment for the program must be made in full two weeks prior to the start of the firstsession. Alternatively, we offer the option of two half payments - one at the beginning of theterm and one at the mid-term point. If you would like to take advantage of this option,please inform the owner two weeks prior to the start of the first session.
You acknowledge and agree that we may increase the fees from time to time in our discretion.
20% late fees apply for payments over two (2) weeks overdue.
A Deposit of $100 is payable at the time of booking. The Deposit is non-refundable. Spots will not be held until this deposit is paid in full.
We will only provide you with a refund of the fees paid if we are unable to continue toprovide the services to you, or if we, in our absolute discretion determine that it is reasonable to do so.
The Student is required to attend their Weekly Session for the Term, unless a session isrearranged for a time that is mutually convenient to both the teacher and the Student- referto the cancellation policy.
The School Readiness will continue for the Term. If a parent decides to withdraw their child throughout the term, they are still responsible for paying the full program fee.
Confirmation of registration for the following term is required by the last week of the currentterm. The deposit (non-refundable) will be used to hold this place.
If not notified, or less than 8 hours notice is given of a child not attending a session, the fullfee for the session/s missed will be charged. If cancellation is made due to sickness, holiday,etc. and at least 8 hours notice has been given, every effort will be made to rebook a make-up session. This ‘make-up’ session must be within the 14 days before or after the usualscheduled session. If this is not possible, full fee for the session/s missed will be charged. Notice of a cancellation must be given via phone call or message to: 0432 260 982. Emails are NOT acceptable for cancellations.

The Parent / Guardian acknowledges that Little Minds in Motion are not accountable orresponsible for the academic success or lack thereof of the Student. By using our services you agree that we are not to be held liable you make based on any ofour services or guidance and any consequence, as a result, are your own.
The Parent / Guardian accordingly waives any and all claims arising out of enrolment in theSchool Readiness Program nor can you hold us liable for any costs incurred by you as a resultof any guidance, advice, coaching, material or techniques used by us.
The Parent / Guardian acknowledges that all materials and resources created by Little Minds in Motion and used in-house or distributed to the Student and/or the Parent / Guardian are the copyright of Little Minds in Motion and agrees not to publish or distribute any portion of any materials created by Little Minds in Motion.
If, in the opinion of any staff member, the Student displays threatening or dangerous behaviour to another student, themselves or any member of staff, the Parent / Guardian will becalled immediately and the Student may be asked to leave the Little Minds in Motion Program.
The Student must be ready to begin sessions on time. NO extra time will be given at the endof a session if a student is late.
The Parent / Guardian acknowledges that it is the responsibility of the Parent / Guardian to inform the Centre Director and / or the teacher of any health concerns/allergies of any kind ofthe Student prior to the beginning of the first Session.
We may take photos or videos during sessions for marketing and promotional purposes. If you do not wish for your child to be included in such materials, please notify us in writing or via the field on this form.
Nothing in these terms is intended to limit or exclude any guarantees, warranties,representations or conditions implied or imposed by law, including the Australian ConsumerLaw (or any liability under them) which by law may not be limited or excluded.
Subject to the extent permitted by law, all terms, guarantees, representations or conditions which are not expressly stated in these Enrolment Terms and Conditions are expressly excluded.

The Tutoring Program will continue for the Term. In the case a student is taken out of their tutoring program (by the parent/guardian), 50% of the remaining term’s fees are payable within 7 days.

Any weekly session that is cancelled with less than eight (8) hours notice will be billed at the normal tuition fee. When eight (8) hours notice has been given, a 50% cancellation payment is required. Notice of a cancellation must be given via phone call or message to: 0432 260 982. Emails are NOT acceptable for cancellations. Only ONE make up session is allowed per term.

The Student must be ready to begin tutoring on time and have all required equipment. NO extra time will be given at the end of a session if a student is not prepared/late.
The Parent / Guardian acknowledges that it is the responsibility of the Parent / Guardian to inform the Centre Director and / or the teacher of any health concerns/allergies of any kind of the Student prior to the beginning of the first Session.
We may take photos or videos during sessions for marketing and promotional purposes. If you do not wish for your child to be included in such materials, please notify us in writing or circle ‘no’ on the Student Form.

Your personal information:

Will not be sold to any other party for whatever purpose. This means your information will not be accessible to any telemarketing or direct mail agencies.
May be used in the course of day-to-day business such as coordination of tutors and students byLittle Minds in Motion.
May be used in the recovery of outstanding payments.
Will be kept absolutely confidential by Little Minds in Motion and will not be given to any otherparty except as agreed by you in the course of business terms and conditions, or if formally requested by law enforcement agencies.
May be used by Little Minds in Motion for following up feedback submitted by you.
